To facilitate an order that is customized to our school’s needs, we are doing a pre-order.

Please take a moment to review the styles and prices, and make your selection on the

form below. Circle the shirt, and size below it and fill in the quantity next to it. Then fill

out the form (at bottom) with your total expenses. Please make the check payable to

"Quest to Learn PTA". Enclose the order form with check in the envelope provided and

have your child give it to the Home Base teacher for the PTA no later than Friday,

Nov 12th. Expect your shirts to arrive in about 2 more weeks.

The shirts will be printed on both sides, back and front, with the design shown below.
